Medical Record Reviewer – Remote

About the role

This contract position involves reviewing inpatient medical records and clinical notes to support AI training and improve data quality. The work is fully remote with flexible weekly hours.

Key responsibilities

• Review medical records, including FHIR data and clinical notes, for AI training.
• Verify that clinical information and claims are supported by the medical record.
• Examine adult inpatient documentation such as HPI, PMH, medication reconciliation, social history, assessments, and plans.
• Identify missing, incorrect, or inconsistent information, including dosing adjustments, medication interactions, and age‑appropriate details.
• Provide feedback to enhance AI models and overall data quality.

Required profile

• MD or DO, board certified in Internal Medicine (or Internal Medicine with a Hospital Medicine focus).
• Strong understanding of inpatient documentation and hospital workflows (admission, daily notes, transitions of care, discharge).
• Experience reviewing clinical notes and raw medical record data.
• Prior work in chart review, clinical documentation improvement, utilization review, or medical coding/QA is a plus.
